Copy of your Marriage Certificate
Your choice of National Cemetery
Vital Statistic Sheet
(available on this website or call and we’ll mail you a copy)
Your choice of disposition (ex. Burial or Cremation)
Having these things on file with Lietz-Fraze Funeral Home will eliminate confusion and the time needed searching for these documents at the time your loved ones make arrangements.
